| Chapter I General Provisions | | 第一章 总则 |
| Article 1 (Legislation Purposes) | | 第一条 （立法目的） |
| These Measures are developed in light of the actual circumstances of this Municipality for purposes of regulating the administration of social security cards in this Municipality, guaranteeing the effective use of social security cards, and improving the levels of social administration and public services of the government | | 为了规范本市社会保障卡的管理，保障社会保障卡的有效使用，提高政府社会管理和公共服务水平，结合本市实际，制定本办法。 |
| Article 2 (Scope of Application) | | 第二条 （适用范围） |
| These Measures shall apply to the application for and the production, issuance and use of social security cards within the administrative region of this Municipality and the relevant services and administration activities. | | 本市行政区域内社会保障卡的申领、制作、发放、使用及其相关服务和管理活动，适用本办法。 |
| Article 3 (Definition) | | 第三条 （定义） |
| For the purpose of these Measures, “social security cards” means the electronic certificates issued by the government in a unified manner for card holders' enjoyment of social security and other public service treatment. | | 本办法所称的社会保障卡，是指政府统一发放，作为持卡人享受社会保障以及其他公共服务待遇的电子凭证。 |
| Article 4 (Card Information) | | 第四条 （卡片信息） |
| The personal information recorded in a social security card shall include the name, photo, social security number, card number and other information recorded on the surface of a card, and the cardholder's other basic information and related application information. | | 社会保障卡记录的个人信息包括持卡人姓名、相片、社会保障号码、卡号等卡面信息，以及持卡人其他基本信息和相关应用信息。 |

| The Shanghai Municipal Population Comprehensive Service and Management Leading Group shall take charge of making overall arrangements for, coordinating and promoting the construction of the social security card information management system, and guiding and coordinating the application of social security cards in the fields of social security, public services and financial services. | | 市人口综合服务和管理领导小组负责统筹、协调、推进社会保障卡信息管理系统建设，指导、协调社会保障卡在社会保障、公共服务和金融服务等领域中的应用。 |
| The municipal public security department shall take charge of verifying the identity information of social security card applicants, taking the lead in the collection of relevant basic information on social security cards, and assuming the specific duties of the Office of the Shanghai Municipal Population Comprehensive Service and Management Leading Group. | | 市公安部门负责核验社会保障卡申请人的身份信息，牵头汇集社会保障卡的相关基本信息，并承担市人口综合服务和管理领导小组办公室的具体职责。 |
| The municipal human resources and social security department shall take charge of verifying social security card applicants' participation in the social insurance of this Municipality, and promoting the application of social security cards in the field of social security. | | 市人力资源社会保障部门负责核验社会保障卡申请人参加本市社会保险的情况，推动社会保障卡在社会保障领域的应用。 |
| Medical care, hygiene and health, education, civil affairs, economic information technology, fiscal, financial and other departments shall, according to their respective functions, effectively provide services and conduct administration related to social security cards. | | 医保、卫生健康、教育、民政、经济信息化、财政、金融等部门按照各自职责，做好与社会保障卡相关的服务和管理工作。 |
| The people's governments of all districts as well as sub-district offices and township people's governments shall cooperate in the effective provision of services and administration related to the acceptance of applications for and the issuance of social security cards within their administrative regions. | | 各区人民政府以及街道办事处、乡镇人民政府配合做好本行政区域内与社会保障卡的申领受理、发放相关的服务和管理工作。 |
| Article 7 (Undertaking Parties) | | 第七条 （承办主体） |
| The Shanghai Municipal Social Security and Citizen Service Information Center (Shanghai Municipal Social Security Card Service Center) (hereinafter referred to as the “Shanghai Information Service Center”) shall be specifically responsible for the construction and maintenance of the social security card information management system, the production and issuance of social security cards, and the organization of acceptance of applications for social security cards and relevant administration work. | | 市社会保障和市民服务信息中心（市社会保障卡服务中心）（以下简称市信息服务中心）具体负责社会保障卡信息管理系统的建设和维护、社会保障卡的制作和发放、社会保障卡申领受理的组织以及相关管理工作。 |
| Community acceptance outlets and bank acceptance outlets shall specifically undertake the acceptance of applications for, issuance, function activation, and reporting of the loss of social security cards, and other work. | | 社区受理网点和银行受理网点具体承担社会保障卡的申领受理、发放、功能开通、挂失等工作。 |
| Chapter II Application and Use | | 第二章 申领与使用 |
| Article 8 (Applicants) | | 第八条 （申领对象） |
| The personnel who have the permanent household registration of this Municipality and the personnel who come to Shanghai from other places within China and participate in the social insurance of this Municipality according to the law may apply for social security cards. | | 本市户籍人员以及依法参加本市社会保险的境内来沪人员，可以申领社会保障卡。 |
| Article 9 (Application Methods) | | 第九条 （申领方式） |
| Applicants may apply for social security cards at community acceptance outlets. When applying for a social security card, an applicant shall submit for verification his or her valid identity document, check his or her name, nationality, ethnicity, gender, photo and other information on the spot, and provide information such as mailing address and postal code. After the information is confirmed, the staff member at the community acceptance outlet shall upload the application information to the Social Security Card Information Management System. | | 申请人可以到社区受理网点申领社会保障卡。申请人申领社会保障卡时，应当交验本人有效身份证件，现场核对本人姓名、国籍、民族、性别、相片等信息，并提供通讯地址、邮政编码等信息，确认后由社区受理网点工作人员将申领信息上传至社会保障卡信息管理系统。 |
| An applicant may also undergo the social security card application formalities with the bank acceptance outlet or through the online service platform. | | 申请人也可以到银行受理网点或者在网上服务平台办理社会保障卡申领手续。 |
| Article 10 (Information Verification) | | 第十条 （信息核验） |
| The public security and human resources and social security departments shall, within three working days after receiving the application information, verify respectively the basic information of the applicant and the information on their participation in the social insurance of this Municipality. | | 公安、人力资源社会保障部门在收到申领信息后3个工作日内，分别对申请人的基本信息、参加本市社会保险的情况进行核验。 |
Where such information passes the verification, the Shanghai Information Service Center shall produce the card. Where such information fails to pass the verification, the original acceptance outlet shall notify the applicant and give the reasons therefor; and if the application is accepted online, the Shanghai Information Service Center shall notify the applicant and give the reasons therefor.
